Co-pay Patients
Your insurance company requires co-payments for services to be paid at the time of your appointment. An administration fee will be charged for co-pays not paid within the same business day. Policy details available.
Self-pay Patients

Late Arrival
If you check in 10 or more minutes late for your appointment, you could be rescheduled at the provider’s request per our policy.
Cancellation Policy
